Sciatica: The Causes, Symptoms and Treatment

Here’s a trivia question for you. What is the longest nerve in your body? Take a wild guess…Yes, the sciatic nerve. This nerve runs from your spinal cord through your hips and buttocks as well as down the back of each leg. It can also pierce the piriformis muscle, a major muscle that starts at the bottom of your spine and stretches across your buttock to your hip.

The sciatic nerve controls the muscles in your lower legs and provides feeling to the thighs, legs and feet. The term “sciatica” refers to the inflammatory pain that travels along this nerve to the above mentioned areas.  In a recent article, we spoke about Cervical Radiculopathy. Sciatica really refers to Lumbar Radiculopathy.

The pain a person feels from sciatica is usually a sign that there is another, much larger problem. Sciatica can be quite painful, but it generally dissipates within a couple of months.  When the pain radiates from a central location, such as the spine, to an outlying area like the legs, it is usually an indication that the nerve supply is at least partially the problem. This is why seeking chiropractic care to resolve this issue makes so much sense.

Causes of Sciatica

Sciatica can develop if a nerve root is compressed in your lower (lumbar) spine. This is usually the result of a herniated disc. These discs consist of two parts, an outer annulus fibrosus and the inner nucleus pulposus. Think of a jelly donut where the annulus fibrosus creates a shell around the nucleus pulposus and helps to distribute pressure evenly across the whole disc. The nucleus pulposus is a mucoprotein gel with the consistency of jelly. It absorbs the impact of your body’s daily activities and keeps your vertebrae separated. 

If the outer shell of a disc tears , the nucleus propulsus can bulge or “herniate”.  The gel contained within then begins to seep out and press on a nerve root. This would cause pain in your back, legs or both. If the damaged disc is in the lower part of your back, numbness, tingling or weakness could be experienced in the buttock, leg or foot.

While a herniated disk is a common cause of sciatica, there are other conditions that could put pressure on the sciatic nerve, including:

Trauma - Any fall or blow to the spine can injure your lumbar area (there are five nerve roots that originate from the lumbar spine) or sacral nerve roots (five nerve roots originate from the sacral vertebrae).
Sciatic Nerve Injury – The sciatic nerve itself could be affected by a tumor or injury.
Lumbar Spinal Stenosis – Spinal Stenosis is the narrowing of one or more areas in your spine, usually in your upper or lower back. If this occurs in the lower spine, the lumbar and sacral nerve roots may be affected.
Piriformis Syndrome - The piriformis muscle is located at your lower spine and connects to each thighbone (femur). Piriformis syndrome is when the muscle tightens or spasms. This puts pressure on the sciatic nerve. This often happens when the sacrum is out of alignment or if you have Spinal Stenosis.
Spondylolisthesis – This often occurs due to degenerative disk disease. It is when one vertebra slips over another. It could pinch the sciatic nerve where it exits the spine.  The vertebra can slip backwards (retro) or forward (antero).

Symptoms of Sciatica

As stated above, pain that travels from your lumbar spine through your buttock and down the back of your leg is the classic sign of sciatica. Pain could be felt almost anywhere along the nerve pathway, but it most commonly follows a path from the lower back to the buttock and back of the thigh and calf. The pain you experience can be a mild ache or a sharp, burning sensation and anything in between. It could also feel like a jolt or electric shock. Other Sciatica symptoms include:

Numbness or Muscle Weakness – This could be experienced along the nerve pathway in the leg or foot. You could experience pain in one part of the leg and numbness in another.
Tingling or a Pins-and-Needles Feeling – Usually in the toes or part of the foot.
Loss of Bladder or Bowel Control – If either of these symptoms is experienced, please seek medical help immediately.

Basic Muscle Tests. These basic tests check your muscle strength and reflexes.

Magnetic Resonance Imaging (MRI). This is probably the most sensitive test for assessing the size of the spinal canal and thus the visualization of spinal stenosis.
Computerized Tomography (CT) Scan. When CT is used to image the spine, the doctor could order the exam with or without a contrast dye. If the dye is used, it is injected into the spinal canal before the X-rays are taken. This dye circulates around the spinal cord and nerves and will appear white on the scan. This will help to clearly delineate the size of the spinal canal.

Treatment

Sciatic pain can usually be relieved through a combination of stretches, massage of the piriformis muscle and most importantly – chiropractic care. Treatment for Sciatica – Avoiding The Causes of Sciatica and Disc Pain

If you are reading this article, We realize that you probably already have sciatica and the information in this chapter may seem like it comes a little too late. Even if you do already have sciatica, there are ways you can help keep it from getting worse, as well as prevent future problems.

So, here’s a list of common things to AVOID as much as possible:

Avoid carrying or lifting something with your arms extended out in front of you. Avoid bending forward at the waist when standing or sitting.

Avoid sitting for prolonged periods of time, particularly in a slouched position or in an unsupportive seat.

Avoid trying to lift something that is very heavy for you.

Avoid twisting repeatedly or while holding something heavy when standing or sitting.

Avoid lifting weights with poor form or body position, or “cheating” a lift by jerking a weight up to gain momentum. Lifting the appropriate amount of weight for your strength level with good form and slow, controlled movements will build muscle much faster than using momentum to lift a heavier weight that you cannot control.

In addition, if you already have sciatica or have recently had an episode of sciatica, it is a good idea to avoid or at least limit high-impact activities like running, jumping, contact sports, off road driving or cycling, snow skiing on uneven terrain, and boating or water/jet skiing on choppy waters.

Now, certain activities like sitting may not always be avoidable. When you simply have to engage in such activities, you can do a lot to reduce the negative effects of them by simply taking frequent breaks.

The breaks do not need to be very long. Just taking a stretch break (or do some of the exercises shown for 30 seconds to a minute every 20 to 30 minutes can go a long way toward preventing problems.

When you need to lift something, it is a good idea to test the weight by lifting with low to m oderate effort to find out how heavy it is rather than grabbing it quickly and pulling up without knowing if it is more than you can handle safely.

When it’s too heavy for you, get help to lift it. It is far better to swallow a little pride and ask for help than to risk an injury that could permanently disable you.

Although commonly used to supposedly prevent low back injuries that can result in sciatica, back belts, whether they be the elastic kind often seen in industry, or the wide leather belts used in weightlifting, are rarely an effective means of injury prevention by themselves.

In fact, some studies on the use of industrial back belts have shown no benefit whatsoever when the back belts were dispensed without any education or training for the users. Back belts are usually helpful as a part of an overall back injury prevention program because they act as a reminder to the user to bend and lift properly (when the user has been educated on the importance of proper lifting techniques).

There are some special back braces that can be used in the course of sciatica treatment that provide stabilization and partial decompression of the spine, but these are usually only dispensed by licensed healthcare providers as a part of an overall treatment regimen. Such doctor-prescribed braces are much more restrictive to movement than the typical industrial back support.

Exercise balls are usually available at any store that carries sporting goods. Beware of extremely inexpensive ones, as these can be prone to leaks or bursting.

Chronic Lower Back Pain: A Simple & Economical Solution

       If you suffer from chronic lower back pain, maybe I can help you avoid making the costly mistakes that I unwittingly committed. Maybe my experience can lead you to an effective pain management solution sooner, rather than later, and save you a boat load of hard-earned cash in the process. My mistake, in a nutshell, was to overlook a simple and economical solution, in favor of much more costly and complicated solutions, extending my suffering unnecessarily. It is this potentially simple and economical solution to chronic lower back pain that I would like to share with you today. To understand and appreciate the solution, you must first understand my story, at least in brief.

       PAIN, lower back pain, became my daily companion after a household move during which I apparently did something to injure a disc in my lower spine, though I had hired movers to do the heavy lifting. MRIs would later show a herniated disc condition. After settling into my new house, I was unable to sleep for more than a couple of hours at a time. My lower back pain would wake me up and make it difficult for me to get back to sleep again. That was probably the worst part of my 20-year ordeal with lower back pain: Being unable to get a good (solid) night’s sleep.

       During the two decades that followed, I tried every approach and type of remedy for lower back pain that I could find. These included (in alphabetical order) acupuncture, chiropractic, epidural cortisone steroid injections, inversion table therapy, massage, medications, pain killers, PLDD (a minimally invasive procedure known as percutaneous laser disc decompression), physical therapy and yoga. All were interesting experiences but none achieved the goal of alleviating my lower back pain for any length of time.
